HHCS works with IRIS consultants, AAA partners, caseworkers, discharge planners, and community partners to connect participants and clients with household, chore, environmental, project coordination, pest-related, and facility/community support services.

Referral Received
Referral details are reviewed and receipt is confirmed so the partner knows the request has been received. HHCS identifies the participant, service need, referral source, and any missing information needed before the request can move forward.
Authorization Review
Coordination
HHCS clarifies the service scope, coordinates the appropriate field partner, supports scheduling, and communicates next steps so the request moves forward with clear ownership and expectations.
Closed-Loop Updates
HHCS reviews the authorization, funding pathway, or private-pay request to confirm the service type, approved scope, dates, and any program-specific requirements before services are scheduled.
Updates continue through scheduling, service status, completion, and next steps so referral partners know what happened after the request was submitted.

IRIS Consultants
Wisconsin IRIS consultants coordinating Chore & Environmental Stabilization Support for self-directing participants under the IRIS waiver structure.
AAA Partners
Utah Area Agency on Aging partners (Salt Lake County Aging Services, Mountainland Department of Aging & Family Services) coordinating chore provider services under the Aging Waiver.
Caseworkers
Caseworkers managing authorization, budget, and documentation across either waiver structure who need a consistent operational contact.
